The management of salivary gland tumors of the palate

Abstract

A study of 22 consecutive patients with palatal salivary gland tumors treated in a structured fashion with primary surgery followed by postoperative radiation therapy, where indicated, and followed for a mean of 7.6 years, showed excellent results clinically, esthetically, and functionally, with no recurrence or metastatic deposits to date. Depending on the type of tumor, the surgery consisted of local enucleation, wide local excision with localized flap closure, or inferior maxillectomy with reconstruction by means of an obturator.

Rapidly progressing palatal pleomorphic adenoma in an adolescent

Minor salivary gland neoplasms are rare in children. Only 5% of pleomorphic adenomas of the minor salivary glands have been reported in patients under 20 years of age. We report a case of a 13-year-old female who presented with a rapidly growing hard palatal mass, confirmed to be a myoepithelial cell-rich pleomorphic adenoma with cytological atypia. Pleomorphic adenomas should be included in the differential diagnosis of rapidly growing palatal masses in children and adolescents. Surgical excision is curative, and long-term follow-up is indicated as recurrence is frequent.

The level of accuracy of aspiration biopsy of salivary gland tumors compares favorably with that of microscopic examination of frozen sections.22 However, lesions should be at least 1 cm deep to be suitable for fine-needle biopsy.19 Both of our patients underwent fine-needle aspiration biopsy of the palatal swelling, and the cytology reports suggested pleomorphic adenoma.
Intraoral minor salivary gland neoplasm: A single institution experience of 80 cases

Abstract.
From March 1991 to February 2001, 80 cases of minor salivary gland tumours were diagnosed in the Department of Oral and Maxillofacial Surgery at the University of Maryland at Baltimore (Baltimore, MD, USA). Data extracted from a retrospective chart review included age, sex, symptoms, site, histological diagnosis, treatment and outcome. Pleomorphic adenoma was the most common benign tumour and makes up 89.5% of all benign tumours. The percentage of malignancy (76.3%) was much higher than that found in other studies. Of the malignant tumours, 54.1% were mucoepidermoid carcinomas. This study differs from many previous reviews that were published by pathologists rather than a surgical unit.

Objective: A variety of surgical procedures can be used to treat malignancies of the hard palate and inferior maxilla. This study was designed to evaluate the efficacy of alveolectomy, palatectomy, and infrastructure maxillectomy in the treatment of cancers in these areas. Methods And Material: A retrospective review of 50 patients who underwent alveolectomy, palatectomy, or infrastructure maxillectomy from 1971 to 1997 was performed. The pathology of these lesions included squamous cell carcinoma (25), adenoid cystic carcinoma (11), adenocarcinoma (6), and others (8). Results: The 5-year survival rate by Kaplan-Meier analysis for all lesions was 85%. The 5-year survival rate for squamous cell carcinoma was 76%, and that for adenoid cystic carcinoma was 90%. The 10-year survival rate for adenoid cystic carcinoma was 75%. There was minimal morbidity associated with these procedures. Discussion: Alveolectomy, palatectomy, and infrastructure maxillectomy are the procedures of choice for lesions in the region of the hard palate. The differences between these surgical techniques are presented, and indications, contraindications, and results for each technique are discussed. (Otolaryngol Head Neck Surg 1999;121:548-52.)
Ultrasonography in the diagnosis of palatal tumors

Objective. The purpose of this study was to evaluate the usefulness of ultrasonography for the preoperative evaluation of palatal salivary gland tumors. Study Design. Eleven surgically treated patients with salivary gland tumors of the palate were examined retrospectively. Intraoral ultrasonic scanning examinations were performed with a 10-MHz transducer. Results. The ultrasonographically well-delimited tumors had a complete capsule histologically, whereas the poorly delimited tumors had no capsule or only an incomplete capsule. The tumors that had a reinforced posterior wall echo were associated with pressure bone absorption, and the tumors that had a reinforced posterior echo were associated with bone destruction. Echograms of the tumors were classified into 4 types on the basis of the internal echoes. Conclusions. Intraoral ultrasonography of palatal tumors can be used to determine the localization and condition of the tumors by close analysis of the echogram. The internal echo pattern on the ultrasonogram of a palatal tumor was found to reflect the pathologic structure of the tumor. Ultrasonography can therefore be a quite useful technique for the preoperative evaluation of palatal salivary gland tumors.(Oral Surg Oral Med Oral Pathol Oral Radiol Endod 1999;87:39-43)
