An emergency department scribe system

Prompted by the need to provide efficient care to large numbers of patients while maintaining complete and legally secure records, a “scribe system” was developed in the Emergency Department of the Decatur Memorial Hospital. By use of a multicopy form divided into space for patient identification information, history, physical, progress notes and diagnoses, plus status-flow sheet for serious ill or injured, a practical nurse, acting as “scribe,” makes notes as physicians obtain history and examine patients. Details of the operation of the system are discussed. In Decatur Memorial, the “scribe system” has improved and expedited patient care, eased the work load, improved record keeping and raised the level of performance of personnel.
